Review: high doses of antipsychotic drugs are no more or less effective than medium doses in people with schizophrenia.

چکیده

Study selection and analysis: Double blind, randomised controlled trials of people with schizophrenia or schizoaffective disorder comparing two or more doses of typical or atypical antipsychotics were eligible. Dose response curves were plotted and the median effective dose, defined as 50% of the maximum response (ED50), and the near maximal effective dose range (ED85 to ED95) was established for each drug. Estimated equivalent doses were based on ED50. Meta-analysis determined whether medium and high doses vary in efficacy. The lastobservation-carried-forward method and intent-to-treat sample were used for the meta-analysis. The possibility of other biases was explored using sensitivity analyses.

Dose response and dose equivalence of antipsychotics.

We review evidence from randomized, placebo-controlled studies of patients with schizophrenia or schizoaffective disorder, which compared 2 or more doses of an antipsychotic to calculate the dose-response curve for each first-generation (typical) antipsychotic (FGA) or second-generation (atypical) antipsychotic (SGA) and as a group (based on dose equivalence). We identified the near-maximal eff...

Olanzapine-Induced Neuroleptic Malignant Syndrome

Neuroleptic malignant syndrome (NMS) is a rare but life-threatening idiosyncratic side effect resulting from neuroleptic drugs. NMS mainly occurs in patients treated with high-potency typical antipsychotics, but rarely caused by atypical antipsychotics. Although NMS is less common with atypical antipsychotic, but it seems that its incidence is rising due to increased administration of such drug...

Strategies for Early Non-response to Antipsychotic Drugs in the Treatment of Acute-phase Schizophrenia

As a strategy for antipsychotic treatment of schizophrenia, monotherapy is clearly optimal when both effective and tolerated. When a patient fails to respond to an adequate dose of an antipsychotic, alternatives include switching, administering a higher dose (above the licensed dose), polypharmacy or clozapine. Clozapine is the only option with established efficacy, but is less manageable than ...

Atypical antipsychotic drugs and the risk of sudden cardiac death.

BACKGROUND Users of typical antipsychotic drugs have an increased risk of serious ventricular arrhythmias and sudden cardiac death. However, less is known regarding the cardiac safety of the atypical antipsychotic drugs, which have largely replaced the older agents in clinical practice. METHODS We calculated the adjusted incidence of sudden cardiac death among current users of antipsychotic d...

Evaluation of the efficacy and safety of pregabalin as an adjuvant to antipsychotics in patients with chronic schizophrenia: a six-week pilot double-blind placebo-controlled trial

Introduction and objectives: Antipsychotics or dopamine receptor antagonists are the major components of treatment but about 10-20% of patients with schizophrenia do not benefit from treatment with antidopaminergic agents, indicating other neuronal systems may be involved in this disorder (2). Dysregulation of both excitatory and inhibitory mechanisms N-Methyl-D-aspartic acid (NMDA) and γ-Amino...
